Dr. Nathaniel Grant struggles to help a young patient in desperate need of a heart transplant. Kate Armstrong, Nathaniels ex-wife and St. Judes organ recovery administrator, labors to find a viable donor. Meanwhile, Nathaniels mentor and boss (Dabney Coleman) may need a transplant of his own.

2. I Make Myself Into Something New

When a liver recipient is back for another chance, Nathaniel has to weigh all the options including the patients sons willingness to help as a living donor. Meanwhile, Kates work with a young Jane Doe hits home, and a new doctor (Rockmond Dunbar), who shares a past with Nathaniel and Kate, joins the staff at St. Judes.

3. Picking Up the Little Things

A young patient Nathaniel transplanted years ago as a small child returns with difficult to diagnose health problems. Meanwhile, Kate works with a prisoner who wants to donate an organ to his dying sister.

4. Mother & Child Reunion

When Nathaniel must give his approval for Jonas to perform a uterine transplant, he questions the risks related to a non-life threatening surgery and weighs the consequences. Meanwhile, Nathaniels heroic measures to keep a baby alive lead him to use a new assisted heart technique from Germany.

5. The Places Youll Go

After years of waiting, a frustrated patient has an illegal liver transplant in Mexico but the botched surgery leaves her worse off, and Nathaniel must now weigh her priority on the transplant list. Meanwhile, Jonas faces a tough personal challenge.

6. Domino Effect

Nathaniel guides his team through what could be a record breaking surgery by attempting a 6-way kidney transplant while also struggling with the ongoing illness of his mentor. Meanwhile, Jonas wife visits and Mary admits she has feelings for someone at the hospital.

7. A Beautiful Day

A teenage killers rampage leaves three dead and Kate saddled with a rookie One Life employee trying to recover organs from the families of the deceased. Meanwhile, Nathaniel affected by the shooting, decides to give his daughter, Thea, a bit more independence.

8. As We Are So Wonderfully Done with Each Other

Nathaniel must deal with the failing health of his mentor while challenged by Dr. Jonas to move forward with a somewhat new and risky procedure. Meanwhile, with his fellowship set to expire, Simons future hangs in the balance.

9. Smile

In Heartland's dramatic season finale, Bart passes away and his widow would like Nathaniel to give the eulogy at his funeral. Meanwhile, panic ensues when it's discovered a former donor had cancer and it has been passed to the recipients of her organs. Kate feels responsible for the disaster and scrambles to find replacement organs. Dr. Jonas uses the opportunity to tell Kate how he really feels about her.
